  3. Enter SPORTFORMA's tax code in the box: 97565030018 (Click to copy).

IF YOU FILE THE PRE-FILLED TAX RETURN (DICHIARAZIONE PRECOMPILATA)

Fill in the 5 per thousand allocation form provided together with your CU (Single Certification) by your employer or pension provider, signing in the box marked "SUPPORT FOR THIRD SECTOR ENTITIES REGISTERED IN THE RUNTS UNDER ART. 46, PARA. 1, OF LEGISLATIVE DECREE NO. 117 OF 3 JULY 2017, INCLUDING SOCIAL COOPERATIVES AND EXCLUDING SOCIAL ENTERPRISES ESTABLISHED AS COMPANIES, AS WELL AS SUPPORT FOR ONLUS REGISTERED IN THE REGISTER" and entering SPORTFORMA's tax code: 97565030018 (Click to copy);
  2. Put the form in a sealed envelope;
  3. Write on the envelope "CHOICE FOR THE ALLOCATION OF THE FIVE PER THOUSAND OF IRPEF" and write your surname, first name and tax code;
  4. Hand it in at a post office (which will accept it free of charge) or to an authorized intermediary for electronic transmission (tax assistance center, accountant…).
